Tax on Retirement Annuity

Which taxes do I pay on retirement annuity disbursements? Fed. Withholding, Medicare, Social Security taxes?

Just federal income tax and maybe state.  Not Social Security and Medicare. You can tell them how much federal and state withholding you want taken out.
