No, Form 5498 doesn't need to be entered on your tax return, but some of the information on it is asked in the IRA contribution section. This form just provides proof of contributions made during the year and your year end value.

 

Here is how to enter your IRA contribution information:

  1. Use the search box to search for IRA contributions.
  2. Click on Jump to IRA contributions.
  3. Check the type of accounts you have, click Continue.
  4. Answer the questions according to your situation. 
  5. Continue through the end of the section.

Sometimes you need the information on the 5498 for example to enter your IRA contributions for the year. But you should have that on other statements. Also note that the 2019 generally doesn't come out until the end of May. So be sure you're not looking at the 2018 form.
