Prompted to enter form 8606-T

I took a distribution from a traditional IRA.  Part was a rollover to a Roth and part was cashed out.  It is prompting me to complete this form which is not ready yet.  Perhaps I made an error in the TT selections.  Isn't this form for non-deductible IRA's?  Im confused.
