Losses from retirement excess contribution

Good Morning

 

Last year I contributed too much to my 401K due to having two employers. 

I did withdraw the excess and there was a loss associated with the withdrawal.

I would like to report the loss this year and asked where to report the loss. I was

given advice on this, however the advice states to report the loss from my 1099MISC.

I never received a 1099MISC, do I need this to be able to report the loss or can I

simply report off of the letter issued by Fidelity which lists the amounts.
