Form RRB-1099 vs Form RRB-1099-R

I have entered the information from Form RRB-1099 (the Blue Form).  I see no place to enter the information from the Green Form (Form RRB-1099-R). I am pretty sure that it need to be entered. Please Advise.

The blue form is your Social Security equivalent and entered in the same place as SSA-1099.

The green form is your annuity or pension and entered as a 1099-R.

The Step by step asks for the box 5, 10 and 11 amounts which are on the Blue Form. It is not giving me an option to enter boxes 3, 4, 7 and 9 which are on the Green Form. Where and how do I enter these amounts? Thanks!
