Box 7 on the 1099-R shows 4D

Can you clarify what the question actually is regarding your 1099-R?

Do I put 4 or D?  The 1099R form, Box 7 shows "4D"

It's two codes, 4 and D.  Select code 4 in the first box 7 selection box and code D in the second box 7 selection box.  In the online version of TurboTax you'll need to mark the box that says that you have a second code to enter.
