401K Distributions

I took two 401k distributions in 2022 from two different 401k plans.

For one distribution I chose a cash distribution. 1099R Box 7 shows    7

For one distribution I chose to rollover directly to an existing Roth (And pay taxes) Box 7 shows G

Turbo Tax combined amounts correctly but put them on 1040 Lines 5a and 5b 

Shouldn't they be on 1040lines 4a and 4b? Why did they appear on line 5 instead of 5.

( Because they are on Line 5 my state tax return will not recognize the amounts for state tax exclusion.)

Lines 5a and 5b are correct. You said that these distributions were from 401(k) plans. Lines 4a and 4b are for IRA distributions. When you see 401(k) plans, think "pensions and annuities".

 

Some states vary in how they handle IRA and 401(k) distributions. What state are you in?
