You just enter the code number that your form uses for box 7.....don't puzzle too much over the different wording
(though the wrong spelling on "non-disability" might be of some concern about who is currently contracted to issue those forms)
I suspect your code number is either a "2" or a "7" , just use whichever of those numbers is on your form....and select that number for box 7 of the Software form.
