You need to ask the 401K plan for the 1099R. You have to get it. It may be you don't have to enter it if it was a direct transfer to a Traditional IRA. You need the 1099R it will have codes on it you need to enter. Maybe you can log into your 401K account and print the pdf. Some places don't mail the tax forms and you have to download it yourself.
