My question is I DID receive a form in 2021 BUT it was FOR 2020.  I did NOT receive a form this year for 2021.  Is this the same thing? In other words, I only received ONE form for the original distribution I took in 2020.  

also, when entered as instructed, it still tells me I need to revisit my 1099-R section which says “Needs Review”.  What am I doing wrong?

Yes, it is the same thing.

If you did Not receive a 2021 Form 1099-R in 2022 then answer No

And -

After completing the Wages & Income section you will land on a screen Did you take a disaster distribution at any time between 2018 and 2020?

Answer NO since you have already completed the entering the 1/3 of the 2020 distribution.

(This will stop the program from taking back to the Form 1099-R section for Review)
