Since the backdoor Roth IRA didn't work, is it correct that my 1099Rs are shown in line 5 (Pensions and annuities) in 1040 instead of line 4? Thanks for your help!
Your 401(k) to Roth conversion is shown on line 5. That is separate from any Traditional IRA to Roth conversation. That has nothing to do with a backdoor Roth.
