If you are seeing boxes 14,15,16 as the state entries on the "software" form during the main part of the interview, then you started using the wrong form-type in the 1099-R section.  Delete that 1099-R and start again.

For the "Online" software, after you start a new 1099-R form, you have to pick the right box on this page.  The Green box shows what sub-form type you've selected:

IF, instead,  you are starting a new 1099-R in the Desktop software, you need to select the CSA and CSF forms on this page.....AND use the interview...do NOT enter your data manually in FORMS MODE !!!:

Now...that being said, if you have filled in the correct boxes on proper CSA- or CSF-1099-R forms Using The Interview, there is sometimes a confirmation/verification page that shows your entries from boxes 12,13,14 now showing up in boxes 14,15,16 .  Yeah....that's OK...TTX moves the values to the boxes it needs them in to report the data properly....and it's only a worksheet that isn't actually filed.  It is also why Desktop users cannot use Forms Mode to fill in these CSA- and CSF- sheets,....as I recall, there are other box entries that get moved around too.
