The Failed Septic Credit is reported on MA Schedule SC.
You will find this in the state interview section by taking the following steps:
- Open your MA State return
- Walk through all of the interview questions until you get to the one that says Massachusetts Credits
- Select Non-Refundable Credits
- In the drop down select Septic Credit
- Enter the information and continue through the interview questions
