Strangely when I logged back in this evening, and went through the same screens as my earlier post it gave me the list of providers and I was able to delete the one I did not want.
To answer the previous question, earlier I was unable to see the provider list on the input page, but when I was running through the review prior to filing, it told me that there was a missing value for the Provider in question and showed me a scrollable image of the actual form. It would not accept zero for the missing value hence the reason I needed to delete the provider from the input screen.
All done now and filed!
