Since 2018 tax returns there is no such thing as "short form" or "long form." There are no more 1040EZ or 1040A forms. Everyone uses a Form 1040 with three possible schedules. Look at your 2018 tax return if you filed one--it is on a Form 1040.
The forms changed again for 2019, but they are all a Form 1040 again. You can use it whether you itemize or use standard deduction.
