Certainly, many have seen that using lower case "wth" for the Utah ID number in the actual original form, fixes their problems, so I doubt the checker is dealing with line 1 at all

....BUT...as long as you did use lower case "wth" on the originating form, I'd bet the real problem for yours is the second form entries,  just to the right side of what you hi-lighted.

You received a W-2 or 1099-R has "Applied for" in the State ID Number box?   No way could you e-file it that way...especially if UT withholding was done for that one.  IF that one did withhold UT taxes, then you'd need to call them now...and see of they have received their UT number since they sent out the W-2 (or 1099) to you, and enter that UT ID number on whatever form was involved there..
