@DMHarley Definitely you should check with the Administrator or Agents handling that Retirment fund.
For some reason, that DS-5014 didn't get implemented, and perhaps you need to send them a new one.
Also, if you have Online access to your retirement account there, sometimes you can make the change in your account yourself.
