You could also just try downloading a new PDF file without using the Add a State...

IF the new PDF has the "Electronic Filing" cover sheets, the dates selected should be on those forms (I'm just not sure if the top-level PDF download still contains those cover sheets) .

IF it does, the info would look like the following with all the critical Direct Debit info on it:
