For a state return, that is a little different since there are 40 states that have personal income taxes.
In Bold letters for the state return it will have Form or Schedule entered at the top of the page and then there will be a Letter or a Number or a combination of a letter and a number at the top of the form or schedule that has to be included with the state tax return. Again, any worksheets or information only pages are not included with the state return.
