After you file

When you phone the Turbo Card system, it will ask you for your 16-digit card number, which you obviously don't have.  Then it says enter your SSN instead.  Did you try that?  If that didn't work, then next time you try their system keep listening further into the menu and don't respond to card number or SSN.  Then it will say "To report a card lost, stolen, or never received press 1".    After pressing 1, then let it go to another menu.  Listen to the choices, and for "never received" press 2.  Continue following the prompts.   I don't know what it will do after that, since I have no way to test it.

If you still can't reach anyone at the Turbo Card, phone TurboTax Support and tell them you are having trouble, and see what they suggest.
