So many inconsistencies between what I sent and what the IRS claims to have received.

Did you enter your retirement/pension income reported on the Form 1099-R?  The IRS received a tax form with income and tax withholding that was not reported on your tax return.

Call the IRS using the phone number on the letter you received to find out who the payer of the income was that you should have entered on your tax return.
