Can you help me figure out what additional forms I am being requested to send? even when both refunds have been accepted?

My state and federal refund have been accepted. I have received my federal refund. However, I have yet to receive my Wisconsin refund as they are asking for additional forms to be sent in. Ive never been asked to do this before so I am wondering if I messed something up in my return? I am having trouble figuring out which forms exactly they are expecting me to send in as well.
