#1 Yes. You only file one federal 1040 return for all your income.
Don't know about your state. Never heard of needing to file more than 1 return for state. The state returns usually start with the federal AGI. Maybe there is some other kind of return you need to file for your LLCs in your state in addition to your state personal return.
