Looks like the form 8911 is available.
1. Click the "Deductions & Credits" tab at the top of the page
2. Scroll down to "Cars and other things you own"
3. Click "Show More."
4. In the drop down you will see "Energy efficient vehicle charging,"
5. Click "Start/revisit"
