You print, sign, date and mail the Form 1040 and all other forms and schedules that have an Attachment Sequence Number located in the upper right corner of the form or schedule. All other worksheets and information only pages are NOT sent with the tax return. Include with the mailed return any forms W-2 and 1099 which have taxes withheld.
