An extension request, Form 4868, for tax year 2016 had to be filed on or before April 18, 2017 for US residents. There are no second extension requests. Payment of any taxes owed had to be paid on or before April 18, 2017 to avoid any late payment penalties.
If your tax return has a tax refund there are no penalties for filing after the due date.
