It's not a bug. Washington is not in the pull-down because Washington does not have state income tax. On your 1099-R box 14 should be blank or zero. When you enter the 1099-R in TurboTax don't enter anything in boxes 14, 15, and 16. Leave them empty. You do not have to enter the state.
