It asks for Employer Identification Number, Social Security of care provider or Other. 

 

I added the EIN and  the mailing info and it doesn't have an AP option for state, only Foreign or US Territory. 


You should use the provider's home of record (HOR) address or, for on-base centers, the Department of Defense (DOD) address. If the provider is a foreign national, you can use your own HOR address. If your dependent care provider is located overseas in an area with an APO address, follow these steps to enter their information in TurboTax: 
For on-base Child Development Centers (CDCs)
If you use a CDC on a military installation, TurboTax support recommends using the DOD address for Child Development Centers. 
  • Provider Name: Department of Defense Child Development Center
  • Address: 111 Constitution Ave NW, Room 1232
  • City: Washington
  • State: DC
  • Zip Code: 20224 
For other dependent care providers
If the provider is a U.S. citizen: Use their U.S. Home of Record (HOR) address. 
If the provider is a foreign national: Use your own Home of Record (HOR) address. 
 
How to enter the foreign provider information in TurboTax
  1. Navigate to the Child and Dependent Care Credit section.
  2. On the screen titled "Let's get some info about your care provider," select Other.
  3. Choose the option for Foreign Provider. This will bypass the U.S. state and zip code validation.
  4. Enter the name of the provider and then their address, city, and country. You can enter an on-base city and country if needed.
