If you have already tried to get the W-2 from your employer and Humana, you can contact the IRS to see if they can step in and help you get the W-2 needed to file your return.
If that does not work then you can file form 4852 with your tax return. You can select the following to enter your wages without one.
- Federal
- Income
- Show more next to Less Common Income
- Start next to Miscellaneous Income
- Start next to Other income not already reported on a Form W-2 or Form 109
- Answer Yes to Other Wages Received
- Yes to Any Other Earned Income
- Employee Compensation that was not reported on a W-2
