When you entered your information in MY INFO did you answer that you made money in another state?
It looks like your employer did not withhold AZ income tax, but if you worked in AZ you are going to have to file a AZ tax return.
You say there is nothing in box 15 of your W-2. Look again at your W-2----is there anything in boxes 16 or 17?
