Most commonly
1) on any W-2, in boxes 15,16,17 (the second half of Box 15 is where the number goes)
or
2) A 1099-R form in the box 14,15,16 area (again, second half of box 15 for your number)
Go back and edit either of those form types you have in your tax file.

But can, less commonly, be in few other 1099-??? form types, if you had any UT tax withholding on them.
