Filed an extension for 2015 taxes. At time of extension paid what was owed. Forgot to file the final return until February of 2016. When is the last date I can file an amendment for this return?

For tax year 2015 with an extension you can file an amended return for a tax refund up through October 15, 2019. After that date if filed for a tax refund, the refund will not be paid.
