I paid my state tax via the online portal, and now I have to send in a Consent due form. I signed most but I am confused on what Title means. I do not have a car or anything of the sorts if that is what it means. Do I print my name and send it in?

Yes, Title just means your position, like Representative, President, etc. (used for corporations and other entities that someone would sign on behalf of).
