How do I upload my 1095-A form?

This is easy to resolve, but you'll need to take action to get that document in before we can finish filing your taxes:
Locate your 1095-A form, which was mailed out at the end of January.

If you don’t have your form, sign in to your HealthCare.gov account and download it, or contact your state's Marketplace. Note: Health Insurance Marketplace plans go by a variety of names based on your state, such as Covered California, Your Health Idaho, New York State of Health, etc.
