Are boxes 16 and 18 blank?
- If so, make sure all 3 boxes, 16-18, are blank. Make sure the fields are completely empty. Put your cursor in the fields and clear them; hit the delete/backspace buttons a few times.
- If there is an amount in 16 or 18, enter the 9 digits (no dashes) from the Payers TIN in box 17.
