Worksheets are not sent to the IRS. The 1099-R box 10 & 11 only pertain to designated Roth distributions whe the box 7 code is B or H.
The worksheet Roth question comes for the 1099-R interview for a Roth distribution determined by the box 7 code.
Do not try to do this in the forms mode since that is not supported and voids the TurboTax accuracy guarantee. Unless you know the relationship between forms that the interview mode automatically takes care of then you should stick to the interview mode.
