1099-R box 10 and 11, and the worksheet

In my 1099-R that I received from my broker, there are no boxes 10 and 11

So when I filled whatever I had and completed my return. When I reviewed the final prepared file , I noticed that on the IRA information worksheet part 5, line 22, it said NO to the question "Opened Roth IRA before 2015".

But this is not true, I had opened it in 2012. Then I saw that in the 1099-R the program prepared , there are also boxes 10 and 11.  When I wrote 2012 to the box 11, the line 22 i talked about above became yes. But I want to make sure nothing is wrong here because my original broker 1099-R doesnt have boxes 10 and 11. 

Do these worksheets or 1099-R get sent to IRS? because if i dont write anything as in my original 1099-R, then line 22 becomes NO. It only becomes yes if I write 2012 to box 11 which i dont even have in my 1099-R
