If you rolled over your Pension to an IRA, the IRA/SEP/Simple box should not have been checked.  A pension plan is not one of these.  Box 2a shows the portion of the distribution which is generally taxable. If there is no entry in this box, the taxable amount was not determined by the payer and it will have to be determined by the taxpayer.  Code G in Box 7 indicates Direct rollover of a distribution to a qualified plan, a section 403(b) plan, a governmental section 457(b) plan, or an IRA.  Distributions with a code G are not taxable.  You shouldn't need a Form 5498 to report your rollover.
