Are you sure you don't mean a corrected 1099-R?
Form 4852 serves as a substitute for Form W-2, Form W-2c, and Form 1099-R, and is completed by taxpayers or their representatives when:
- Their employer or payer does not give them a Form W-2 or Form 1099-R.
- An employer or payer has issued an incorrect Form W-2 or Form 1099-R.
If you are creating the substitute form, be careful. The IRS expects you to make a reasonable effort to get the form and prescribes the steps to take. The idea is not to use one just because you don't want to wait for it.

To enter Form 4852
You can file a conventional paper return that includes IRS Form 4852 if all efforts to obtain a W-2 (1099-R) from your employer have failed. Here's how to fill one out:
- In your TurboTax program, search for W-2 (1099R) and then select the Jump to link in the search results to get to the W-2 entry screen.
- Fill out your W-2 (1099-R)as best you can using the information from your pay stub.
- When finished, proceed through the interview. Eventually you'll get to the Do any of these uncommon situations apply to this 1099-R screen.
- Check the Substitute form box near the bottom.
- Check any other box(es) that apply to your W-2 (1099-R) before selecting Continue.
- Eventually you'll come to a screen which will ask if you want to complete Form 4852 for wages paid. Answer Yes and carefully follow the onscreen instructions.
Returns including Form 4852 can't be e-filed per IRS regulations.
