To file a technically correct and complete tax return you should file an amended return showing the Form 1099-R distribution and rollover.

If the distribution code shown on the 1099-R is a “G” or “H” then you are probably less likely to get any correspondence from the IRS for not reporting the rollover distribution on your tax return in the event that you decide not to amend your return. 

If the distribution code is anything other than a “G” or “H”, then it is not reporting a direct rollover from one institution to another and you do need to file an amended return in order to report the rollover.
