Explain what you mean by "from a 401a *and Roth* to simple IRA"? What does "and Roth" mean?
Are you saying that the 401(a) was a designated Roth (401(a) Roth) account? A Roth cannot be rolled to a SIMPLE IRA.
A Traditional 401(a) to SIMPLE IRA is not taxable. What code is in box 7 on your 1099-R for the 401 distribution? What is in box 2a? Is box 2b checked?
