On 1099-R, box 4 Federal Income Tax withheld: $2,359.31 

On 1099-R, box 7 Distribution Code # "7"

On 1040, line 64, this amount was not included. (I calculated all the (Federal) taxes withheld from the W-2s and 1099's and $2,359.31 appears to be missing from the total.)
