Is the 1099-B Worksheet complete? I had sales which require entries in Boxes A, D, and E. The worksheet displays transaction data for only 2 of those boxes.

Using Windows Premier  Desktop version. I've entered data using step-by-step for boxes A, D, E for sale of mutual fund shares and calculations appear to be correct.  But the 1099-B wks displays detailed information for only two of those boxes (E and A) . Where can I see the detailed information for the Box D item?
