Should I have received a form from my receiving 401(k) plan after a rollover?

My old plan sent me a 1099-R, and I am asked if I contributed Roth. I did not for the old plan, but I did in the new plan.  Do I answer yes or no for the old plans form, if I don't have a form for the new plan?
