Retirement tax questions
No. That should have been reported on your 2015 tax return. If you did not report it then, you do not need to amend, but can file a stand alone 8606 form with the 2015 contribution on line 1. Any prior nondeductible contributions from the last filed 8606 line 14, on line 2. The total of 1 & 2 on line 3 and 14. And mail it to the IRS.

Enter 8606 in the find box and choose the 2015 form and instructions.
