The Deluxe edition would create Form 4972.
To enter the lump sum distributions, here are the steps (online):
- Sign into your account, select Take me to my return
- To the right upper corner, in the search box, type in “lump-sum distributions”
and Enter
- Select Jump to lump-sum distributions
- Follow prompts
