In either TurboTax Desktop or Online, enter the information as if it were reported on an IRS form 1099-B.
Under Retirement Plans and Social Security, click to the right of IRA 401(k) Pension Plan Withdrawal (1099-R).
At the screen Do any of these situations apply to you?, select I need to file a substitute 1099-R.
