IRS states in Pub. 970 that a rollover Roth IRA contribution does not need to be entered in your tax return. Save Form 5498 reporting the rollover for your tax records.
If the contribution is for the beneficiary, and you are the recipient, the beneficiary would keep the documentation for their records.
