If it is a Joint and Survivor Annuity: (Meaning the payments continue to your wife if you pass away), you must enter both your age and your wife's age as of the annuity start date. As you proceed through the questions, there will be a box to enter ages for both the primary recipient(you) and the beneficiary(your wife). Here you would enter both of your ages.
In answer to your other questions you asked. You are correct in all four questions. You will need to reenter the plan cost as you record the annuity information.
