consider preparing 1040-X manually.
5329 penalty goes on 1040-X Line 10 and Schedule 2 Line 8.
So your penalty amount should be in Line 10 Col. B correcting Col. C.
An updated schedule 2 and updated 5329 should be attached,
but, do not attach your old 1040 nor your revised 1040 because the Form 1040-X reflects any changes there and becomes your new tax return.
Mail the three forms to the address shown for your location in the most recent Instructions for Form 1040-X.
